Payments

Please plan to take care of co-payments, co-insurance and any outstanding balances at the time of service.  Co-payments or deductibles must be paid at the time of visit.  We accept debit cards, Visa and Mastercard.

If we are not a participating provider with your insurance plan, or your provider does not cover services provided, you will be responsible for any charges incurred.  We will provide you a receipt for your insurance company.    

Any check returned from the bank will result in an additional $20 charge.

Always notify our office of any change in name, address, phone or insurance information.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card with you to every visit so we can ensure that our records are accurate. Also please notify us if you have any change in your name, phone, address or insurance as soon as it occurs.

Our physicians participate with many insurance plans. In regards to insurance where we are a participating provider, co-pays and deductibles are due at the time of services. In the event that your insurance coverage changes to a plan where we are not a participating provider, the balance is your responsibility.
